1. Upon inspection of the place the following may be requested:
(A) 3 no. copies of layout plans of the place (layout plans shall not be required with renewal application unless alterations have been carried out)
(B) Periodic Inspection and Test Certificates completed by a competent Electrical Engineer in respect of the Electrical Installation, Fire Alarm System and Emergency Lighting if applicable (the Electrical Installation Certificate shall be required at max. 3 year intervals whereas the fire alarm system and emergency lighting certificates shall be required annually)
(C) a certificate to confirm that public liability insurance is in force (including limit of indemnity).
(D) in circumstances where the proposed place is considered to be unusual in terms of type and/or location a suitable and sufficient risk assessment, relating to health and safety of persons in or about the place, prepared by a competent person may be required.
2. I understand that the place has no recent or continuing religious connection and I have obtained any necessary permissions regarding use of and access to the place.
3. I understand that the place may be inspected for suitability before approval is granted and, if this application is successful, may be subject to subsequent inspection.
4. I further declare that, if approval is granted, I will comply with the conditions of approval (Appendix A) attached to that grant of approval.
This application may be made by any person. If successful the applicant will be the holder of the Approval. (The Approval, if granted, will be valid for a period of 1 year from the date it is granted.)
